Removed no_strict_aliasing.
Renamed index to idx to avoid shadowing BSD strings.h [kwilliams]

g++ -O2 works OK with gcc 4.4.0
eg/q_test produced some problems to chase (see Changes.txt)
Passes qhulltest --all
